PUD due to H.Pylori

Protocols

Clinical Features of H.Pylori PUD:

·       An ache or burning pain in your stomach (abdomen)

·       Stomach pain that may be worse when your stomach is empty.

·       Nausea.

·       Loss of appetite.

·       Frequent burping.

·       Bloating.

·       Unintentional weight loss.

 


Investigation:

1. Upper GI Endoscopy : gastritis or biopsy taken

2. Non invasive test:

Serology

Urea breath test

Faecal Antigen test 

3. Invassive test : 

Histology 

Rapid urease test


Rx:
1. General treatment:

a. Avoid NSAID 

b. Avoid cigarette smoking & alcohol 

c. Avoid spicy foods 

d. Free from mental stress


2. H.Pylori Eradication Therapy: 

1st Line (Tripple Therapy)

Tab. Helicon Kit { lansoprazole(30mg)+amoxicillin(1gm)+clarithromycin(500mg)}

1+0+1...............10-14 days 


2nd line if not improved: 

Syp. Peptocid (Bismuth) 

2 tsf .......3 times daily .......14 days 

Cap.Lansec (Lansoprazole) (30mg) 

1+0+1...............14 days 

Tab.Fimet (metronidazole) (400mg) 

1+1+1..........14  days 

Cap.A-tetra (Tetracycline)(500mg)

1+1+1+1 ...............14 days 



3. Surgery : if indicated 

1. partial gastectomy 

2. Vagotomy & drainage
