HELLP Syndrome

Protocols

Diagnostic Criteria: HELLP

  • Haemolysis (H)
  • Eleveted liver enzyme(EL)
  • Low platelete count (LP)
  • Epigastric pain 
  • Features of pre-eclampsia or eclampsis


Investigations:

  • CBC 
    • Low Hb 
    • Low platelete 
  • SGPT: Increase
  • Serum Creatinine : increase
  • Urine RME 
    • proteinuria

Rx: ( patients need ICU support) 

  • Diet : Soft 
  • Fluid & salt restriction 
  • Bed rest 
  • 02 inhalation stat & sos 

If DIC / Haemorrhage : 

  • inj. K-one MM (10mg) 
    • 1 amp daily ....3 days 
  • Blood Transfusion 

If pg <34 weeks : 

  • Inj . Roxadex (dexamethasonse)(5mg) 
    • 2.5 ampl ..I/M stat & 
    • 2.5 amp ...I/M after 12 hours of 1st dose 

If pulmonary edema : 

  • Inj.Lasix (20mg) 
    • 1 amp ....I/V stat & BD  (8 am & 4 pm)
