Typhoid Fever/Enteric fever

Clinical Features:

• 1)High fever of up to 104 degrees Fahrenheit
• 2)Headaches
• 3)Abdominal pain, constipation then perhaps diarrhea later
• 4)Small, red spots on your abdomen or chest (rose-colored spots)
• 5)Loss of appetite and weakness

Other symptoms of typhoid fever include:

• 6)Body aches
• 7)Bloody stools
• 8)Chills
• 9)Severe fatigue
• 10)Difficulty paying attention
• 11)Agitation, confusion, and hallucinations. 

Investigation:

• 1)CBC
  
• 2) Urin R/E
• 3)Blood culture -1st week
• 4)Widel test- 3rd day & 2nd week
• 5)MP & ICT For malaria
• 6)Usg of W/A
• 7)Stool culture - 2nd week
• 8)Urine culture -2nd week

Rx: (uncomplicated case)

Tab. Denver([post id="399" title="Cefixime"]) (400 mg)

• 1+0+1…..14 days
  

OR 

Tab. Zimax ([post id="401" title="Azithromycin"]) (500mg)

• 0+0+1..…...7 days
  

OR

Tab.Rutix(ofloxacin) (200mg)(15mg/kg/day)

• 1+0+1……...5 days

patients toxic হলে /Treatment failure(complicated case)

Inj. Oricef (ceftriaxone) (2gm) 

• 1 vial 1/V- BD…...14 days

Cap. Seclo(omeprazole) (20mg)

• 1+0+1....( খাবার ৩০ মি আগে)......10 days

if fever:

Tab. Napa([post id="449" title="Paracetamol"]) (500 mg)

• 2+2+2 ( জ্বর> ১০০f )
  

Supp. Napa (500 mg)

• ১ টা পায়ু পথে (জ্বর > ১০২ F হলে)
  

Tab.Santogen(multivitamin) 

• 1+0+1.....15 days

Typhoid Carrier Rx:

Tab. Neofloxin (ciprofloxacin) (750mg)

• 1+0+1.............4 weeks

OR

Tab.Cotrim(Cotrimoxazole) (400+80mg)

• 1+0+1........................6-12 weeks 

if not improved then surgery : 

• Cholecystectomoy

Typhoid vaccine নিতে হবে 

inj.Vaxphoid (0.5ml) (age > 2 year)

• 1amp I/M stat single dose ...every 3 yearly

Advice:

• বাইরের তৈরী খাবার খাবেন না।
  
•  কম সিদ্ধ করা খাবেন না।
  
•  খাবার আগে ও পরে ভালো সাবান দিয়ে হাত ধৌতকরণ করবেন।
  
• বাড়ির বা বোতলের পানি পান করবেন।
